If Your Dog Has Died
If your dog has died you are required to notify Animal Control in writing, either via online dog services or by completing the Dog Registration Rebate Form and returning it to Animal Control.

You may be entitled to a partial refund. Refunds are calculated from the date Animal Control is notified in writing.
Animal Control may request you to supply a copy of the veterinary certificate, if available.
Once the notification is processed by Animal Control the registration tag will be deactivated.
