Many of the forms in the list below can be completed online and will then be automatically sent to Council. Alternatively, the PDF versions can be emailed, posted, or delivered in person to our Blenheim or Picton Customer Service Centres.

Club Licence or Renewal of Club Licence Application

Complete an application for a club licence or renewal of a club licence.
Manager's Certificate Application

Complete an application for a manager's certificate or renewal of a manager's certificate.
Notice of Management Change

Complete a notice of change in management. Includes new certificate holding manager, temporary or acting manager, and termination/cancellation of manager appointment.
Off Licence or Renewal of Off Licence Application

Complete an off-licence or renewal of off-licence application form.

Section 100(f) Certificate Application
